Referrals

NHS e-Referral Service

We use the NHS e-Referral Service for referrals. If you are referred to hospital you will receive all the information necessary to book or rearrange your appointment at a hospital of your choice from the surgery.

Once we have referred you your referral goes to the Booking and Scheduling office. If there is a problem with your referral or you need to re-arrange an appointment please call them using 0161 778 2233.

If you wish to speak to one of our secretaries about a referral please call the surgery and choose option 3.

NHS e-Referral Tracking

If your doctor has referred you to a specialist for further care you will be asked to book your appointment using the ‘NHS e-Referral’ system.

You will receive a letter from us within 2 weeks giving details of how to book your appointment and any passwords you may need.

You can track your referral using the NHS app. Information about signing up for online services can be found here.

Alternatively to track your e-Referral you will need the following details to log into the e-Referral website:

  • Your date of birth
  • Reference number – this will be on the first page of your letter
  • Password – this will be on the second page of your letter

Self-Referral

PhysioDirect

PhysioDirect provide Physiotherapy intervention for individuals ages 16 and over who are registered to a GP in Heywood, Middleton, and Rochdale.

Our services are available Monday-Friday between 8am-7pm and our admin services are open between 8am-4pm.

Services may include:

  • Home exercise plans.
  • Advice and education surrounding your condition and symptoms.
  • Gym based exercise sessions.
  • Individualised rehabilitation programmes.
  • Group based rehabilitation classes: early ankle, early lower limb, advanced lower limb, shoulder, Osteoarthritis, total knee replacement, Graded Exercise and Ladies groups.
  • Post-surgical and fracture rehabilitation.
  • Mobility assessments.
  • Balance and proprioception training.
  • Acupuncture (at some sites).
  • Provision of walking aids.
  • Forward referrals onto other services.
  • Injection therapy.

Some treatments are restricted to specific areas, clinics, and physiotherapists. More information can be found here.

 

Podiatry Direct

Podiatry Direct provide a comprehensive foot health service that includes the diagnosis, management and treatment of lower limb and foot dysfunction and their associated pathologies.

Patients meeting the access criteria will be routed to the appropriate podiatry team member for assessment, diagnosis, treatment and management, that may involve referring on to colleagues within the service, for example, Nail Surgery Specialist, Biomechanical & Gait Assessment, High Risk Podiatrists and to other agencies, for example Physiotherapist, District Nurses and GP.

If you're aged over 16 years of age you can self-refer. Parents or guardians can also refer there children under the age of 16 years.

Please note that self-referral is not available for routine nail cutting and skin care, including corns and callus in healthy patients, verrucas, bunions, or fungal nail infections.
