TRAVEL ADVICE

Some holiday destinations make recommendations on which vaccinations travellers should have before travel. The following vaccinations are available free of charge from your GP:

· Poliomyelitis (Given as combined Diphtheria, Polio, Tetanus)
· Typhoid
· Hepatitis A
· Cholera

You should contact us at least 8 weeks before travel using this form to ensure that the vaccine has sufficient time to protect you but also to provide your GP surgery sufficient time to arrange an appointment. If you leave booking an appointment too late you may find that there are no available appointments prior to your travel date and may have to access these vaccinations through a private clinic at a cost to you.

All other vaccinations including Yellow Fever, Meningitis ACWY and Japanese Encephalitis are not available on the NHS. We do not offer these vaccinations at our surgery, so you will need to go to a private clinic to access any non-NHS travel vaccinations.

If you require a certificate or form completing to prove that you have had the above NHS supported travel vaccines, then this can be completed by your GP surgery but is likely to incur a charge as completing certificate/forms is not covered by NHS services.

It is recommended that you arrange a consultation regarding travel at least 8 weeks before departure if possible. Vaccines need time to take effect and some may require a course over several weeks.
