Sick (Fit) Notes

Urgent advice: If you've been advised by the hospital to obtain a sick note from your GP

If you are currently under a hospital specialist or reecntly discharged from the hospital with advice to obtain a sick note from your GP - return to the hospital to ask them to issue an appropriate sick note for the required duration. Hospitals are able to issue sick notes for the period they consider necessary.

Following the government’s imposition of a new GP contract, the British Medical Association has advised GPs to stop providing unfunded activity in general practice. As of Monday 6th January we will not provide sick notes on behalf of providers who have the provision and ability to provide appropriate sick notes themselves.

If you've been off sick for 7 days or less

If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. We call this self-certification.

If you've been off sick for more than 7 days

To request a sick note:

  • request a sick note using PATCHS
  • phone or visit the surgery from 10am to 6pm

If you have not seen a doctor at the practice and we have had no information from a medical professional about your illness, you will need a GP appointment to get a sick note.

 

If you need an additional sick note

To request an additional sick note if you are still sick:

  • request a sick note using PATCHS
  • phone or visit the surgery from 10am to 6pm

Fit notes can be back-dated so it is not necessary to make an emergency appointment to renew your certificate. If you attend an emergency appointment for the purpose of getting a medical certificate you will be asked to rebook.
