How to register

If you wish to register as a patient please check your postcode using the "Practice Boundary" tool below.

If you are within our boundary proceed to the "Register as a New Patient" section and complete a Registration Form (GMS1) and a New Patient Questionnare.

Register as a New Patient

If you wish to register as a patient we require that you complete a Registration Form (GMS1) + New Patient Questionnaire. These are listed below.

Once you have submitted the forms, you should attend one of our surgeries with some of the required information/documentation listed below.

  1. Photo Identity (e.g Passport, Driving licence, European Union Card,)
  2. Proof of Address (e.g letters, bills)
  3. Your previous address and previous Dr’s surgery name.
  4. Your NHS number
  5. If you are from outside the UK you will also need to bring in your EHIC card/proof of entitlement to NHS treatment.

If you forget to bring any of these items it may mean that we are unable to process your request for registration.

It will speed up the process if you are able to bring in a photocopy alongside the originals.

Please note that these documents can only be accepted between the hours of 09:30 and 16:30. If you bring your forms outside these hours we will only accept them if you also bring a copy of the above documents with your originals. For one of our reception staff to copy your documents they will have to come out of reception (the copier is upstairs) which is only acceptable when we have enough staff members to cover the demand at the desk.

Temporary Registrations

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

Your Named GP

As part of a national programme introduced April 2015, all practices are required to provide all their patients (including children) with a named GP who will have overall responsibility for the care and support that the surgery provides to them.

At the Junction  Surgery, all of our patients have been registered to a named GP partner and they will be the GP with this responsibility. If you do not know who your registered GP is, please feel free to contact the surgery and a receptionist will be happy to inform you. You may continue to request to see any of our GPs for your appointments and so you should see no change in the day to day running of the practice or your care.
